Client intake forms for your website, bookings and clients
Short answer
Forms is where you make the questions you ask people and where every answer comes back. Start a form from a template, such as Client intake, or from blank. Edits save on their own and go live without a Publish button. Then put the form where you need it: on your website as a link or embedded on a page, in a booking type, or sent to a client by email and in their client portal. Each answer is one record: you read it on the form's Answers tab and in the place it came from. Aventide never turns an answer into a client by itself. Forms have no plan limits and use no AI credits.
How Forms work
Where can one form live?
In as many places as you need, at once. On the form's Share tab, Website gives you its link and an embed code for your site. Booking adds it to a booking type, so clients answer it when they book. Clients sends it to one client, by email and in their portal. You can also share the plain link anywhere else, such as a newsletter or a social bio, and those answers arrive like website answers. A form in more than one place says so, for example Used in 3 places, and edits show in all of them. Every answer keeps the version of the form it was given on, so an old answer still reads the way it was asked.
Where do the answers go?
To the place the form was answered. A website answer arrives in your Team inbox as a card your whole team sees, counted in the inbox line of your Daily Brief, and the person who created the business gets an email saying someone answered, without the answers in it. A booking's answers sit on the booking, in its detail and on the calendar. A client's answers go straight onto their record: the Forms panel in their room, and a line in their History such as Answered Client intake. Every answer is also listed on the form's Answers tab with who sent it, when and from where. Download CSV puts them all in a spreadsheet, with times in your business's time zone, and Delete answer removes an answer from every place at once.
Does an answer make someone a client?
No. Proof attaches, typing suggests, and only you create a client. An answer joins a client by itself in two cases: it came through the private link you sent that client, or with a booking made with an email that belongs to exactly one client. A typed email on a website form only suggests: the Team inbox card reads Looks like Fernwood Bakery, with Link and Not Fernwood Bakery, or offers Make a client for someone new. Make a client adds them at the Lead stage and never makes a second client for an address you already have. Someone new who books becomes a contact, and a paid booking from someone new comes with a card asking whether to make them a client.
How do booking questions work?
A booking type asks a form. In Calendar, open a booking type and pick a form for its questions, make a new one there, or choose No questions (name and email only). Clients answer after they pick a time and before they pay, and the page skips the form's name and email questions, since the booking asks for those. "My onboarding process is a discovery call, a contract PDF, an intake form in Notion, and a first session," a coach wrote in April 2026. Here the intake form rides on the booking. Answers show on the booking's detail and on the calendar. If a type had its own questions before, they move onto a form the first time you open it, and past bookings keep their answers.
Can I put a form on my own website?
Yes. Embedded, the form has no Aventide frame, so your site stays the brand, and it grows or shrinks to fit its questions. The plain link opens the form on its own page at app.aventide.ai, with your logo, colors and heading font, and one small Powered by Aventide line under it. A long form splits into pages with a step count, and it works on a phone. A bot check, a hidden trap field and limits on how often one person can send keep spam out of your Team inbox.
How to make a client intake form in Aventide
Start from Client intake
Open Forms, click New form, give it a name and pick Client intake. It starts with nine questions in two parts. About you asks for their name, email, phone, company and website. The project asks for their goals, what success looks like, any deadline, and the best way to reach them, by email, phone or text.
Change the questions
On the Build tab, edit any question or add your own from 13 answer types: Short answer, Paragraph, Email, Phone, Website, Number, Date, One choice, Several choices, Dropdown, Scale, Yes / No and Tick box. Only show when hides a question until an earlier answer matches, a note can sit between questions, and Add a page splits a long form. Preview shows it as your client will see it.
Share it where clients will see it
On the Share tab, send it to a client, add it to a booking type, or copy the link or the embed code for your website. A client you send it to gets an email in your business's name with a private link that can only send answers, and the form appears in their portal, marked done once they've answered. If they reply to the email, it reaches whoever on your team sent it.
Read the answers
Open the client's room to see their answers in the Forms panel, or the form's Answers tab to see everyone's. Each sent form shows Waiting, Opened or Done; no email tells you when a client answers, so the panel is where to look. Remind sends the email again, and Copy link gives you the link to paste somewhere else. Nothing reminds a client unless you click. To close a form, choose Stop taking answers in its menu.
Form templates to start from
Forms comes with six starting points, and each one is an ordinary form you can change. Client intake asks nine questions about a new client and their project. Project inquiry is for your website: their name, email, company or website, what they need, a rough budget, an ideal start date and how they heard about you. Brand questionnaire asks about their business, ideal customers, the words that fit their brand, brands they admire, what to avoid and the colors they use. Before our call asks what they want to cover and anything to read first. Feedback asks how happy they are on a scale of 1 to 5, what went well, what could have gone better, and whether you may share their words as a testimonial. Blank starts empty.
What Forms cost
Forms are on both plans, like every feature: Starter is $39 a month and Pro is $99 a month, each with unlimited businesses and unlimited team members. Forms have no plan limits and use no AI credits: building a form, sending it and reading its answers cost nothing extra. Doing things yourself never uses credits. Only the AI's work does, so asking Aventide to draft a reply to a website answer in the Inbox uses 10 credits. Starter includes 40,000 credits a month and Pro 120,000. Prices and credit costs as of October 17, 2026.
Questions about Forms
What is a client intake form?
A client intake form is the set of questions a new client answers before the work starts: who they are, how to reach them, what they want and by when. In Aventide it's a form you send from the client's room or ask on a booking type, and the answers go onto that client's record. The Client intake template starts with nine questions you can change, so you can send it the day someone says yes.
Can a form take signatures, files or payments?
Not today. Forms don't collect signatures; anything a client must sign is a contract, which has e-signature. There are no file-upload questions yet, and payment is taken on your booking page or by invoice, not inside a form. A Tick box records that someone agreed to your wording, and that wording is kept with the answer. If you need files or payments inside a form, a general form builder is a better fit for now.
Can I ask health questions?
Aventide's templates never ask about health, such as conditions, medications, injuries or pregnancy, and there are no consent-form templates. If you write a question that looks like a health question, the builder warns you and suggests leaving it out; the warning doesn't stop you saving, so the choice stays yours. Forms are for client work, not medical or clinical records.
Can I connect Google Forms, Typeform or Tally?
Not yet. Today, the forms that file their answers onto clients, bookings and your Team inbox are the ones you build in Aventide. If you keep another form tool on your website, its answers stay in that tool. To move a form, rebuild it in Aventide from a template or from blank, then put it on your website, your booking types or your clients; its answers land in Aventide from then on.
Do my clients need an account to fill in a form?
No. A client opens the private link in your email, or the form in their portal, and answers. That link belongs to one client and one form: it can only send answers, never shows anything back, and stops working after 60 days or when you stop it. Sending the same form to the same client again reuses that link. A website form is open to anyone with the link, with a bot check in front of it.
What happens if I edit a form while someone is filling it in?
Their answers aren't lost. When they click Send answers, the page loads the new version with their answers still filled in and says "This form was updated while you were filling it in: check your answers and send again." Nothing is saved until they do. A page left open for more than a day gets the same check. Every answer keeps the version it was given on, so your Answers tab shows each question as it was asked.
Is there a limit on forms or answers?
There are no plan limits on how many forms you make or answers you collect. A form holds up to 100 questions on up to 20 pages. To keep out spam, one person can send the same website form up to 10 times an hour, and your website forms take up to 200 answers an hour in all; past that, the page asks people to try again in a few minutes. These limits are as of October 17, 2026.
Does Aventide's AI write or fill in my forms?
No. Nothing in Forms uses Aventide's AI: you write the questions, and your clients answer them. Aventide's AI works elsewhere, for example in Think, where you talk your business through, in Do, which makes finished documents, and in the Inbox, where it can draft a reply to a website answer for you to approve. That draft reads only what the card shows, not the whole answer.
